the ones i own and think are pretty radical are:
andrew wk - who knows
anegls & airwaves - start the machine
armor for sleep - a comprehensive guide to touring
bastards of young (great documentary about the new jersey punk/hardcore/emo scene)
blink 182 - urethra chronicles 1 & 2
riding in vans with boys
drive thru records - vol. 1 & 2
jimmy eat world - believe in what you want
new found glory - the story so far
park - weathering a summer
i've been looking for:
braid - killing a camera

Shit Happens- Every Time I Die
Still Reigning- Slayer
Centuries of Torment- Cannibal Corpse
This Is Who We Are-As I Lay Dying
Majesty-The Black Dahlia Murder
The last three are fairly recent but I feel they are some of the best band DVD's I've watched. The production on them are top notch and gives great insights into the bands. BDM's Majesty puts Shit Happens to shame in terms of wild antics in my opinion.

Best band DVD ever. I'm still waiting on part two
I expect it to be twice as crazy as the last one was, it would be a letdown if they tamed it down a bit.
