I've heard that they mean the same thing but I disagree.

i look at post hardcore as bands that sound kinda like saosin and screamo is bands like greeley estates

I've always seen post-hardcore as bands like Thursday and At the Drive-In. I have no idea what sreamo is though.

post hardcore - husker du, fugazi, quicksand, rival schools, bear vs shark, seaweed, etc etc etc


screamo - off minor, ...who calls so loud, alessa, she died real pretty , bravo fucking bravo, etc etc etc
