For sex, options #1 and #2

For shelter if I'm homeless and cardboard boxes just aren't cutting it, options #3 and #4.

For a date, options #1 and #2.

For being stranded on an island with one other person and must resort to cannibalism in order to survive, options #3 and #4.

For a wife whose appearance will help net her many job opportunities, thus enabling her to help provide us and our children with a comfortable life, #1 and #2.

For a wife whose physical condition will help net thousands of dollars in disability checks per month, thus enabling her to help provide for me and our children for maybe 10-12 years before they eventually die from heart disease courtesy of the awful genes and eating habits they got from their mother, #3 and #4.

So, it depends.

All other things being equal I'd say thin (though an option 3 or 4 who's a charismatic, rocket scientist billionaire beats a former high school cheerleader any day).

As for thin being unhealthy...no it's anorexic/bulemic that's unhealthy. If you're skinny because you exercise and eat controlled portions that's very healthy. It's if you simply don't eat or just purge anything you do eat that's unhealthy. When I put option 1 I'm not talking holocaust survivor skinny.
