In my opinion, the following bands never lived up to their debuts:

Cartel
fun.
Saosin (EP)
Scary Kids Scaring Kids (EP)
Senses Fail (EP)
Taking Back Sunday

Regarding the previously mentioned bands (Finch, Say Anything, and Funeral for a Friend), I've never found any of their albums particularly interesting, so I wouldn't put them in this thread.

There are a number of bands that only have on album and it's a great one, but that list would be pretty large.

I love De-Loused In The Comatorium, but nothing else by The Mars Volta that I've heard really does anything for me.

Panic! at the Disco. Fever was so good and the follow ups were "meh" at best.

Those two are my favorites by each band. They're definitely the most well-formed efforts from each band, at least.

I think Cartel is a pretty classic example also (as long as we aren't counting EPs).

I agree with Cartel; the EP wasn't anything special.

The live show is still solid (it helps that they trimmed the fat), but I just can't see them topping the creativity and execution of Chroma.
